POSITION SUMMARY:  The Case Manager will provide case management, outreach, and support services for older adults in Baltimore City. The case manager is responsible for conducting client assessments, assisting clients with meeting requirements to access grant and loan funding for home repairs & modifications, maintaining accurate records on all ongoing cases, completing monthly, quarterly, and annual reporting, and attending meetings and outreach events for the program. The Case Manager should be self-motivated, interested in assisting older adults, and interested in promoting and supporting the Central Baltimore community. The Case Manager is responsible for developing specific strategies to expand the program.

This position is temporary for one year and could be extended based on need & funding.

The selected candidate will work 40 hours per week and receive salary plus benefits including short and long-term disability, supported health care, 401k and dental.

Civic Works also trains Baltimore residents for employment in the healthcare and green job industries.

 

HOUSING UPGRADES TO BENEFIT SENIORS (HUBS) Housing Upgrades to Benefit Seniors (HUBS) is a collaborative project that places social workers and case managers based at five sites in Baltimore. They facilitate aging-in-place services for older adults by connecting them to home repair resources that improve the quality of their lives through accessibility improvements, rehabilitation, weatherization, and environmental hazard remediation. While providing application assistance for these resources, the HUBS staff members also assist clients accessing other benefits by doing full-service needs screening and connecting them to those resources.

 

JOB RESPONSIBILITIES:

  • Identify, contact, and assist low-income older adults with issues such as home repair, home modification, or weatherization services;

  • Connect older adults to other housing services, which include handyman services, tax credits, foreclosure prevention, and tax sale prevention through the completion of city, state, and nonprofit applications;

  • Assist older adults in applying for resources or benefits that make their homes safer and more affordable;

  • Maintain records and report activities to funding sources

  • Collaborate with community, city, state, federal, and other social service agencies to support program goals.

  • Support other HUBS partner organizations with managing their caseload (when necessary).

  • Partakes in HUBS social worker monthly meetings and triages difficult cases as needed with support of colleagues.

  • Advocate with agencies or persons to help clients receive appropriate benefits or services;

  • Expand existing programs and identify additional resources;

  • Uphold high organizational standards.

  • Performs related duties or responsibilities as assigned or requested. 

Qualifications

EDUCATION and EXPERIENCE:  

  • Bachelor's degree in Social Work, Psychology, Gerontology, or other related field.

  • At least 1 year of case management experience is required.

  • Experience  working with older adults.

  • Passion for engaging older adults and promoting the dignity of the human spirit.

  • Experience navigating database software (preferred).

  • Proficient knowledge of Microsoft Word, Excel (preferred).

 

SKILLS AND ABILITIES:

  • Drivers License (A driving record will be requested as well)

  • A Drug test and a Criminal Background check will be required.

  • Demonstrated ability to creatively solve problems.

  • Demonstrated ability to work independently and collaboratively.

  • Ability to adapt to a changing work environment.

  • Strong Organizational skills.
